Bariatric Surgery by other Surgeons / Institutions

If you had a bariatric surgery by another surgeon or institution and you have relocated, your bariatric surgeon has retired or moved, or you wish to continue your follow-up care elsewhere, our Bariatric Center offers a comprehensive care plan which includes regular follow-up with a bariatric surgeon and a dedicated Bariatric nurse and dietician.

Prior to your appointment please authorize release to our office of all your medical records and particularly the operative report and discharge summary of your original bariatric surgery, follow-up notes with your bariatric surgeon, any work-up, procedures or additional hospital stays for treatment of complications and any available laboratory or X-ray work-up performed after your original surgery.
