Mission Statement

The mission of the Comprehensive Diabetes Outpatient education program of the Diabetes Care Center at St. Francis Hospital and Medical Center is to provide comprehensive diabetes self-management education, training, and support to individuals with diabetes and their families. This information will be based on the American Diabetes Association’s Standards of Care. The ultimate goal of this program is to encourage interventions, both self-management and medical treatment alterations, that will lead to improved diabetes control, a decrease in diabetes related complications, and a heightened sense of patient empowerment.

The goals of the Diabetes Care Center are to:

• Serve as a resource for area medical providers in assisting patients with their diabetes management
• Assist participants in the achievement of optimal glucose control
• Develop individualized diabetes treatment regimens that lessen or prevent diabetes related complications.
• Provide a resource for meeting the educational and emotional needs of individuals with diabetes and their families.
• Provide tools for self-care skills.
