Program and Services

The Diabetes Care Center offers a four-day program of diabetes treatment and education. Components of this full program are listed below.

  1. Physician assessment/evaluation. This may or may not be included, as determined by the wishes of the referring physician.
  2. Nursing assessment/evaluation.
  3. Nutritional assessment/evaluation. Provision of an individualized meal plan.
  4. Comprehensive diabetes education which includes:
    • Individualized instruction in the use of monitoring equipment and insulin delivery devices.
    • Explanation of diabetes
    • Healthy eating guidelines, including portion control and restaurant choices
    • Signs, symptoms, and treatment of hyperglycemia, hypoglycemia
    • Medications
    • Complications/standards of care
    • Sick day guidelines
    • Safe exercise guidelines
    • Foot care guidelines
    • Stress management
  5. Consultations with a podiatrist, social worker and exercise physiologist, as needed.
  6. Recommendations for future care according to the American Diabetes Association Standards of Care.
  7. Laboratory and other diagnostic tests as indicated and ordered by the diabetologist.
  8. Daily fasting, pre-lunch and 3 p.m. blood glucose tests at center with adjustments in diabetes medication as needed.
  9. Interaction and support of other group members with diabetes.
  10. Daily breakfast and lunch supervised by a registered dietitian.
  11. Diabetes literature available in English and Spanish.
  12. Parking reimbursement of up to $4 per day.
  13. Comprehensive medical/nursing/nutritional reports are sent to the referring physician. Includes all laboratory and ancillary testing data.
  14. Telephone contact, as needed, for a six-week period for blood glucose reporting and medication adjustments. Referring physician may elect to adjust the diabetes medication himself.
  15. A six-week follow-up appointment. A progress report that includes the results of repeated laboratory tests is sent to the referring physician.
