Referral Process
Patients wishing to be referred to the Diabetes Care Center Program should start the process by contacting their primary care provider to request a referral to the Diabetes Care Center. It is the responsibility of the patient and the primary care provider to verify insurance coverage for the Diabetes Care Center program. The referring physician’s office calls the Diabetes Care Center (860-714-4402) to refer the patient and obtain the appointment.
