Our Team
"Working Together, Rebuilding Lives" is our motto and our pledge at Mount Sinai Rehabilitation Hospital. Each patient works with an interdisciplinary care team headed by a physiatrist, a physician who specializes in physical medicine and rehabilitation. The team may include:
- Family members
- The family physician
- Rehabilitation nurses
- Physical, occupational and speech therapists
- Case managers
- Social workers
- Recreation therapy specialists
- ...as well as other healthcare professionals
Teams meet weekly on each patient to evaluate progress and update goals. Patients and their families are involved in decisions about care and plans for discharge.
